Richard Jack Ballard, 52, of Waipahu, a Navy retiree, died in Waipahu. He was born in Ohio. He is survived by wife Angeles "Carmen"; stepson Frederick Dela Cruz; stepdaughter Carolyn Dela Cruz; parents Arthur and Rosseta; brothers Danny, Eddie, John and Bill; sister Sue; and two stepgrandchildren. Visitation: 9 a.m. Monday at Mililani Mortuary-Waipio, makai chapel. Services: 10:30 a.m. Flowers welcome.
