75Lance H. Miyashiro, 40, of Wahiawa, an employee at Mililani Pet Store and the Mai Tai Bar in Ala Moana Center, died. He was born in Honolulu. He is survived by parents Thomas and Carole, brother Todd and sister Dawn Chinen. Visitation: 9:30 a.m. Thursday at Mililani Mortuary-Waipio, makai chapel. Services: 10:30 a.m. Burial: 11:30 a.m. at Mililani Memorial Park. Casual attire. No flowers.
