Chad Eric Takashima, 40, of Honolulu, an employee at McCorriston Miller Mukai MacKinnon and at Heaven on Earth Salon & Day Spa, died in the Queen's Medical Center. He was born in Honolulu. He is survived by fiancee Memorie Tiapula, hanai son Nathanael, hanai daughter Kolohe and sisters Susan Okuhara and Kathy Okunami. Visitation: 5 p.m. Thursday at Borthwick Mortuary, mauka chapel. Services: 6:30 p.m. Private scattering of ashes at a later date. Casual attire. No flowers.
