74Phyllis Jean LaForge, 76, of Laupahoehoe, Hawaii, a retired nursing administrator for Borgess Medical Center in Kalamazoo, Mich., died in Hilo Medical Center. She was born in Malden, Mo. She is survived by sons Mark and Greg, and two grandchildren. Visitation: 9 a.m. Jan. 16 at Hilo United Methodist Church. Celebration of life: 10 a.m. Additional services in Michigan. Casual attire.
