Nichole Perreira-Pagatpatan, 34, of Kahului, a teacher assistant at Kamehameha Schools Paukukalo Preschool, died in Kaiser Permanente Moanalua Medical Center. She was born in Honolulu. She is survived by husband Ken Pagatpatan; son Clayton Pagatpatan; daughters Tianni and Kuulei Kaiwi and Kenie, Nevaeh and Pomai Pagatpatan; parents Terry and Sharon Perreira; brother Brian Perreira; and sisters JoAnn Maldonado and Tesha Perreira.
